Post-CABG

Post-CABG (open heart surgery) Graft lesions

Post-CABG (open heart surgery) Graft lesions occurs when the graft fails post the procedure, resulting in ischemia.
Failure of the graft can occur due to multiple reasons such as:

  • Progressive Atherosclerosis – plaque build-up
  • Intimal hyperplasia – abnormal accumulation of cells
  • Thrombosis – blood clots
  • Altered coagulability of the blood
  • Technical error or poor run off
  • Platelet adhesion
  • Patient related – age, diabetes, LV dysfunction, renal insufficiency

Medical management for prevention of graft failure

Surgical option

Percutaneous Coronary Intervention (PCI) Stenting

Procedure

A beam of light directed at the artery gets reflected & scattered
  • Scattered light causes glare, which OCT filters off
  • Small amounts of reflected light that does not get scattered can be detected
  • Used to form the image of coronary artery
  • Cardiogists can see detailed inside view of artery

Applications

Assessing the plaque formed
  • Stent struts coverage
  • Stent apposition assessment
  • Stent restenosis evaluation
  • PCI guide & optimization
